Is Super Easy (And A Little Fun)

At Feller Orthodontics, we understand that choosing an orthodontist is an important decision. We are always excited about meeting new patients during the first office visit. Patients over the age of 18 will be asked to complete a new Patient Packet. If under age 18, the accompanying parent or legal guardian will complete this packet of information.

Your initial appointment will consist of a thorough examination and a discussion of potential treatment options. This visit will give us insight into your orthodontic needs.

During the initial examination, we will address the following questions that cover the basics of your orthodontic treatment. We encourage you to ask questions as well, we don’t bite!

Is there an existing condition that orthodontics can correct?
Is treatment needed now, or should treatment be delayed until appropriate growth, tooth eruption or other factors have occurred?
What treatment procedures will be used to correct the problem?
Do any teeth need to be removed?
How long will treatment take?
How much will it cost?
What are my payment options?

Is Filling Out The Forms Before You Come In, Seriously

At Feller Orthodontics, we offer a complimentary and thorough Head, Neck, Oral and Occlusal Exam. In order to provide the highest possible care to you or your loved one, we require an accurate, fully completed and signed HEALTH HISTORY FORM. For your convenience, these forms are customized for either an adolescent or an adult. Identical forms are also available in Spanish.

These forms are ready for you to download and then to type directly in your responses,right from your computer or tablet. Once you’ve filled out your information, please email the form to us. You can also print them and bring them completed with you to your appointment, or even fax them to us. Please use the below email address or fax number to send us your forms.
