At Feller Orthodontics, we offer a complimentary and thorough Head, Neck, Oral and Occlusal Exam. In order to provide the highest possible care to you or your loved one, it takes an accurate, fully completed and signed HEALTH HISTORY FORM. For your convenience, these forms are customized for either an adolescent or an adult. Identical forms are also available in Spanish.
